What To Bring To Your Appointment
Here is the list of required documentation to either upload prior to your appointment or bring with you to your appointment. If you are ill on the day of your appointment, please reschedule. We will need all the required documents to complete your application. Please make sure that you upload your documents or bring them with you to your appointment.
-Social Security card(s) or official documentation with social security numbers (or Permanence Residence/Green Card) for everyone living in the household.
-The most recent copy of your gas and/or light bill. If you are a propane client, you should bring a statement from your propane company establishing that your account is in good standing.
-Proof of household income for 30 days for anyone 18 years of age and older. Check stubs MUST show the name of the employee, pay dates and gross income. OR, If you receive Social Security or Disability benefits, you MUST provide your current Social Security/SSI Award Letter. OR, Unemployment Summary of Benefits
If you had no income in the previous month, zero Income can be verified with our Zero Income Form which will be provided in the office. We also require a Wage Verification from the Department of Labor.
